POLAR WAVE CARE & GAME READY

Do you have a prescription from your doctor for a Polar Wave Care or Game Ready?

Here are the steps to follow before your upcoming surgery.

Rental of the Polar Wave Care (Breg):

Please send the following two documents to soins@jehangermontreal.com:

1. The PDF consent form below (printed and filled out)

2. A copy of your prescription

It is important that you send us the two documents as soon as possible so that we can contact you regarding the rental of a Polar Wave Care. If you wait until the week of your surgery, it is possible that we will not have any available machines to rent.

The Polar Wave Care can be picked up 1 to 2 days before your surgery in one of our offices (5545 rue St-Jacques, Montréal, H4A 2E3 or 4650 Desserte Sud autoroute 440, suite 210, Laval, H7T 2Z8).

If you wish to prolong your rental, please contact the office that prepared your initial invoice.

Rental of the Game Ready (Heal Faster / Évolution Médical):

Please send the following two documents to soins@jehangermontreal.com:

1. The PDF consent form below (printed and filled out)

2. A copy of your prescription

It is important that you send us the two documents as soon as possible. Évolution Médical will contact you to plan out the rental of the Game Ready (payment/delivery).

The Game Ready will be directly delivered to your house and a return label will be provided.
